Step 2: Check Your Insurance & Payment Options
Visit our Insurance & Payment page for details about accepted plans and self-pay rates. If you are unsure whether your insurance covers care with us, please call your insurance company directly to verify that we are in-network.
Step 3: Complete New Patient Registration
You will need to fill out our required forms before your first visit. These forms can be downloaded from our Forms & Policies page or completed electronically through our Patient Portal. This helps us get to know your health history and goals before your appointment.
Step 4: Schedule Your First Appointment
Once your forms are submitted, our front desk will work with you to find a convenient time for your first visit. We offer both in-person and telemedicine appointments. Directions and parking information can be found under Patient Resources.
Step 5: Prepare for Your First Visit
